Registration for USANZ 2012 is now open   

There are two different ways of registering for USANZ 2012—online or manual registration.

 

Online registration

Please note online registrations can only be paid for with a credit card (MasterCard, Visa, American Express, Diners).

 

Attendees from Christchurch ASM 2011 are eligible for a discount on the registration fees. To register at this discounted fee, please click here. Registrations will be checked for eligibility and only delegates who paid to attend the Christchurch ASM will be accepted. Non-eligible registrants will be sent an invoice for the difference between the discounted fee and the standard rate.  Please note that this discount is only available until 31 January 2012.

Please note manual registrations can be paid for via credit card, electronic funds transfer (EFT) or Australian cheque. Please ensure you have read the attached terms and conditions before submitting your completed registration form.

Registration entitlements


USANZ delegates, accredited trainees
& retirees

Fulltime registration (member & non-member)

  • Attendance Welcome Reception

  • Attendance at scientific sessions

  • Access to the industry exhibition

  • Morning tea, lunch, afternoon tea on  all meeting days                   

  • Satchel including program and meeting handbook

  • Note that the gala dinner is not included in the registration, however is available at an additional cost of AUD 175 (incl. GST). The registration fees have been reduced to reflect this change.

 

Day registration

Day delegates will be entitled to the following entitlements on their nominated day:

  • Attendance at scientific sessions on nominated day

  • Attendance to post session drinks on nominated day (Tuesday and Wednesday only)

  • Access to the industry exhibition

  • Morning tea, lunch, afternoon tea on nominated day

  • Satchel including program and meeting handbook

  • Note that the gala dinner is not included in the registration, however is available at an
    additional cost of AUD 175 (incl. GST)
     

ANZUNS delegates

Fulltime registration (member & non-member)

  • Attendance at Welcome Reception

  • Attendance at ANZUNS and USANZ scientific sessions

  • Access to the industry exhibition

  • Morning tea, lunch, afternoon tea on all meeting days

  • Satchel including program and meeting handbook

  • ANZUNS Bard Educational Evening on Monday 23 April. Note that the gala dinner is not included in the registration, however is available at an additional cost of AUD 175 (incl. GST).

 

ANZUNS program only registration (member & non-member)

  • Attendance at ANZUNS scientific sessions only

  • Access to the industry exhibition

  • Morning tea, lunch, afternoon tea on ANZUNS meetings day

  • Satchel including program and meeting handbook

  • Note that the gala dinner is not included in the registration, however is available at an additional cost of AUD 175 (incl. GST)

Terms & conditions

Registration cancellation policy
All cancellations must be notified in writing to MCI Australia. Cancellations received on or before 12 November 201 will be refunded in full, less 25% of the fee to cover administration costs. No registration refunds will be made after this date. As an alternative to cancellation, your registration may be transferred to another member of your organisation, subject to a AUD33 processing fee.

Payment

  • All registration and function prices are quoted in Australian dollars and include GST.

  • Payment must accompany your registration form.

  • Your registration cannot be confirmed until payment is received.

  • Payment must be received by the due date to receive the discount, otherwise the next level of payment will be charged.

  • Cheques or drafts must be in Australian currency, drawn on an Australian bank and free of all charges.

  • Cheques should be made payable to Urological Society of Australia and New Zealand.

  • Cheques should be mailed to:

  • Payment by credit card is also acceptable. Please note that debits to your credit card will appear as MCI Australia on your statement.

  • Faxed registration forms are acceptable only if payment is by credit card.

  • Payment may also be made by electronic funds transfer. Please ensure the amount transferred is equal to the total due INCLUDING any bank charges.

  • To assist in the allocation of your payment, please fax transfer advice to +61 7 3858 5499 immediately after payment to ensure prompt allocation of your payment.

  • Please ensure that your name and invoice number are included on the transfer notice.

Insurance
Registration fees do not include insurance of any kind. Neither USANZ nor MCI will accept liability for damages of any nature sustained by participants, or loss or damage to their personal property. It is recommended that you take out a suitable travel insurance policy when you register for the meeting and book your travel.

Privacy statement
Your name and contact information, including electronic address, may be used by parties directly related to the event such as the organisers and approved stakeholders, for relevant purposes such as promotion, networking, and administration of this, and future events of this type.
In addition, your name, organisation and country/state of origin may be published on the delegate list which is provided to delegates, exhibitors and sponsors at the event.
